University Park Healthcare Center is a 88-bed Skilled Nursing Facility specializing in post-acute psychiatric care.
We are currently looking for a *Registered Dietitian*! This position advises the facility on opportunities related to dietary activities: Reviews and suggests changes in dietary functions and administrative procedures in the dietary department.
*ESSENTIAL DUTIES AND RESPONSIBILITIES*
* Work in collaboration with management to make sure nutrition screening and assessment of all residents are completed upon admission.
* Monitor care plans. Checking to ensure high-risk residents are frequently monitored.
* With facility dietary staff, coordinates all phases of nutrition care including nutrition assessment, care planning, monitoring, education, and counseling of patients in a variety of settings.
* Assist with menu changes and QAPI program as needed.
* Conduct quarterly assessments and annual assessments as needed.
* Do quarterly or monthly reviews of dialysis residents for facility.
* Monthly review of residents with pressure ulcers
* Follow up on dietary consult orders.
* Review residents with significant and insidious weight changes (insidious has no formal definition and so is up to the discretion of the RD).
* Monthly kitchen audit.
* Conduct in service for dietary and nursing staff as requested.
* Expected to stay abreast of current research on nutrition related health conditions and adjust individual care plans as needed to improve success in addressing nutrition related issues.
* Other duties as assigned.
*REQUIREMENTS*
* Bachelor’s degree in Dietetics or related field
* National CDR Registered Dietitian – Commission on Dietetic Registration
* Minimum 4 years experience as a Dietitian for skilled nursing facilities
* Possess the ability to supervise and work well with others
* Demonstrates the ability to perform managerial tasks
* Possess high standards of personal cleanliness and neatness.
* Demonstrates knowledge of basic nutrition
* Possess the ability to prepare meals per the available menu
* Must have transportation to travel to different facilities
* Bilingual English/Spanish highly preferred
