Job Description\n

American Traveler is hiring an experienced RN for a Palliative Care position in a home health and hospice setting, requiring at least 2 years of acute care experience and an active OR RN license.

\n \n Responsibilities\n

• Provide care in a home health and hospice environment focused on palliative care
• Day shifts scheduled Monday through Friday, 5x8 hour shifts
• Responsible for case management of 12 to 15 hospice families
• Utilizes Epic and Remote Client EMR platforms for documentation
• Comprehensive patient assessments, wound care, pain and symptom management, and hospice documentation required
• No local candidates accepted
• Daily responsibilities include skilled nursing care, education of patients and caregivers, and interdisciplinary team collaboration
• Includes case management, discharge planning, and timely clinical documentation
• Supports bereavement services for families after patient death
• High level of team coordination expected
• First-time travelers with prior hospice experience are welcome to apply

• Active OR RN license required at time of consideration
• Minimum of 2 years recent acute care nursing experience
• Prior hospice experience required for first-time travelers
• Current BLS certification
• Ability to perform comprehensive assessments, wound care management, and symptom management
• Experience with Epic EMR preferred but not required
