The Mission of Veterans Place is to empower veterans' transition from homeless to home, ending the cycle of homelessness, and to assist all at-risk veterans to become engaged, valuable citizens who contribute to their communities. Veterans Place is a CARF Accredited organization, following the ASPIRE model, to provide quality, value, and optimal outcomes of services to our clients.
Position Summary:The Transitional Housing program at Veterans Place provides up to two years of residential support for Veterans who are homeless or at risk of homelessness. The Transitional Housing Manager (THM) is the person with primary responsibility for working with Veterans Place case management to set and track programmatic goals, establish roles for transitional housing staff, and ensure progress toward independence and permanent housing for veterans.
Position Responsibilities:
Service Activities:
• Work directly with VA Liaison to coordinate veteran care and grant compliance.
• Oversee veteran care from start to finish, ensuring that iVETS model and CARF ASPIRE best practices in housing services are followed.
• Directly supervise the Assistant Case Manager Coordinator to ensure that shifts are covered.
• Ensure that Assistant Case Managers provide services in accordance with the ASPIRE model and VA GPD requirements.
• Assist the case management team in the creation and implementation of Individualized Service Plans (ISP), which incorporate employment, educational and income/financial and recovery goals for veterans.
• Support the Director of Clinical and Recovery Support Services in the development and implementation of an outreach strategy in support of the Transitional Housing program.
• Support and assist Case managers in the development and implementation of resident's service fees
• Coordinates with Facility Manager and Executive Director to ensure compliance with VA GPD regulations concerning transitional housing and successful audits and inspections.
• Coordinates with Facility Manager and Executive Director to ensure compliance with CARF ASPIRE regulations concerning transitional housing and successful certification audits.
Clinical Activities:
• Set standards for Assistant Case Management team regarding interactions and support provided to each veteran
• Assist intake assessments and intakes in collaboration with Case Management staff and other provider organizations including the Veterans Administration.
• Assist the coordination of referrals of Veterans to the Veterans Administration and other community-based providers for additional services based on needs and eligibility (this includes expanding the existing referral network as necessary).
• Coordinate with assistant case managers to ensure they are engagement techniques with veterans that are evidence based and demonstrate good clinical practice.
• Implement legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations.
• Participate in Treatment Team and Clinical Team meetings to ensure continuity of care.
• Ensure ample group and resource options are available to all veterans.
• Ensure proper input of veterans into HMIS.
• Ensure that group activities are provided, occur on time and follow the developed structure and content.
• Facilitate group services to be identified in cooperation with the Executive Director and the Director of Clinical and Recovery Support Services.
• Coordinate group services with the Director of Clinical and Recovery Support Services and the Executive Director to ensure that groups provide a range of information and utilize evidence based practices and materials.
Milieu Activities:
• Direct supervision of Assistant Case Manager Coordinator, who directly supervises Assistant Case Managers (24/7 staff)
• Oversee the delivery of services across the Milieu including but not limited to the following;
o Urinalysis testing
o Hand scans
o Curfew
o Daily Checks
o Inspections
o Contract adherence
o Rounds
o Transportation
o Events/Activities
o Room searches
o Groups
o Resident Council
o Community Meetings
Program Development:
• Complies with work rules and policies.
• Complies with facility safety rules and takes appropriate corrective action to ensure work is performed in a safe manner.
• Oversees transitional housing in the community to ensure the facilities are complying with contract requirements.
• Takes part in community events and outreach related to veterans
• Coordinate supportive services with applicable providers including the Veterans Administration
• Oversee the delivery of services within the scope of this program and make recommendations to the Veterans Place Executive Director and leadership team for further development and refinement of the program.
• Work closely with the VA Grant and Per Diem Liaison to ensure a high quality of services.
• Work with the Case Management team to develop community building activities during non-business hours.
• And other duties as requested.

Qualifications:
• Bachelor's Degree or equivalent experience required.
• Experience working directly with the homeless population strongly preferred.
• Must have a valid driver's license.
• Veteran Status preferred.
• Passing a pre-employment drug and alcohol screening is required and a background check will be conducted
