Position: Psychiatric Mental Health Nurse Practitioner (PMHNP)

Job Description:

The Psychiatric Mental Health Nurse Practitioner (PMHNP) will provide psychiatric evaluation and medication management services at a nursing home on the west side of Chicago. The PMHNP will work in coordination with the nursing home counseling services to provide psychiatric services to adult clients with behavioral health issues. This position will be supervised externally by a psychiatrist with whom there is a Collaborative Practice Agreement established upon hire. Liability insurance and collaborating physician costs are both covered by the company.

Initially, services will be nursing home-based, with the opportunity to expand to telehealth services. The PMHNP will work in collaboration with the Medical Director to coordinate client care, assist with insurance pre-authorization, and manage case-loads.

Job Requirements and Qualifications:

Minimum Required Education:

  • Master's or Doctorate Degree in Nursing with a focus on Psychiatric Nursing.
  • Graduation from a state-certified advanced practice program specializing in psychiatry/mental health.

Minimum Required Experience:

  • At least 1-2 years providing direct client care to persons receiving treatment for psychiatric disorders.
  • Ability to work with adult populations.
  • 3+ years of experience as a psychiatric provider.
  • Experience in telemedicine.

Licenses / Certifications:

  • Must be certified by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • An unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Illinois.
  • Must have a valid DEA number and unrestricted DEA license in Illinois.
  • A state-issued Controlled Substance Registration (CSR) in Illinois.
  • Registration with the state Prescription Monitoring Program (PMP) in Illinois.
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g., Medicare, Medicaid, SCHIP).

Other Skills Required:

  • Experience in delivering high-quality, client-centered care and ability to document and navigate EHR independently.
  • Ability to maintain strict client confidentiality.
  • Effective communication and interaction skills with all stakeholders including adults, family members, staff, grantors, partner agencies, and community members.
  • Proficient computer skills, including working knowledge of Microsoft Office Suite.
  • Ability to work effectively on a multidisciplinary team.
  • Strong language skills for reading, analyzing, and interpreting professional documents, and writing reports and correspondence.
  • Independent decision-making ability.
  • Teamwork and leadership skills for training and directing colleagues.
  • Excellent organizational skills and ability to make independent decisions.
Working Conditions:
  • Full-time, equal to 40 working hours per week.
  • Flexible work schedule with hours between 7:00 am and 9:00 pm.
  • Continuously meet minimum required hours of direct services to clients each week, allowing for documentation, meetings, trainings, and supervision.
Essential Duties and Responsibilities:
  • Provide psychiatric services to clients in various clinical settings during scheduled hours, including outpatient psychiatric evaluations and medication management services.
  • Assess, diagnose, and treat adults experiencing symptoms of behavioral health disorders.
  • Prescribe, recommend, or discontinue drugs and treatments in accordance with the Collaborative Practice Agreement and state regulations.
  • Collaborate with counselors, social workers, or other treatment team members to coordinate client care.
  • Ensure timely and accurate completion of medical records, including assessments, treatment plans, progress notes, outcome measures, and billing entries.
  • Counsel clients during appointments as necessary.
  • Design and participate in the development of individualized treatment plans, incorporating best practices into treatment interventions.
  • Order laboratory or diagnostic tests as needed for clients.
  • Communicate with guardians, relatives, and significant others about clients' conditions and treatments when clinically appropriate.
  • Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies as required.
  • Promptly return calls and emails (within 24 hours or sooner if clinically necessary).
  • Make independent decisions based on protocols utilizing national guidelines for treatment interventions.
  • Maintain customer relations with all agency staff and clients.
  • Submit at least 10% of medical records to the supervising Psychiatrist for biweekly review.
  • Ensure accurate billing for services provided in compliance with laws and regulations.
  • Participate in education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures.
