Job Description Summary

The role of a therapist in the Physical, Occupational, and Speech Therapy departments is pivotal in implementing an individualized home health therapy program that encompasses a broad spectrum of responsibilities including assessment, treatment planning, and patient education. The successful candidate will actively foster positive relationships with other healthcare professionals and staff within the home health environment, working towards achieving the functional goals of each patient.

Essential Job Functions/Responsibilities

1. Patient Care and Treatment Planning

• Develop and implement patient-centric care plans based on comprehensive assessments and including the involvement of significant others where appropriate.
• Deliver direct patient care, focusing on evaluating functional needs and outcomes, while collaborating with other specialists to adjust care plans as necessary.
• Lead in the initiation of skilled care episodes, focusing on potential developmental assessments (specific to PT role).
• Maintain high standards of therapy practice within established norms of care.

2. Interprofessional Communication

• Establish a fruitful communication channel with other professionals and support staff to ensure positive patient outcomes.
• Participate actively in interdisciplinary team meetings to review and revitalize patient care plans.

3. Compliance and Documentation

• Adhere strictly to the hospital and departmental policies, procedures, and quality improvement programs, maintaining an emphasis on safety, environmental protocols, and infection control standards.
• Ensure compliance with all relevant laws, regulations, and agency policies.
• Maintain precise and current patient documentation via Foundations Senior Services and Your Tailor Made Senior Services EMR systems, adhering to HIPAA standards.

4. Professional Development

• Facilitate personal and team growth through active participation in educational programs, in-service meetings, and workshops.
• Stay abreast with the latest developments in the field through consistent review of professional literature.

5. Leadership and Community Engagement

• Provide guidance and support to therapy assistants and other staff members, acting as a valuable resource.
• Undertake leadership roles to foster physical therapy awareness in the community, while also collaborating with the management team to enhance patient care quality and meet agency objectives.

6. Administrative Responsibilities

• Work closely with the intake department to expedite new patient assessments.
• Participate in continual training and education to uphold current competencies in therapy practice.
• Communicate professionally with all stakeholders, including office staff and field workers.
• Adhere to the directives outlined in the Foundations Senior Services Policy Manual to ensure consistent compliance.
