Position Purpose:

The Case Manager for the Women's Residential Treatment Program in South Los Angeles plays a vital role in providing comprehensive care coordination to minority and low-income individuals, including pregnant and postpartum women and their children. This position is responsible for maintaining a high standard of care in alignment with the Substance Abuse Prevention and Control (SAPC) and Drug Medi-Cal (DMC) program guidelines, while supporting JWCH’s mission to improve the health and well-being of underserved populations in Los Angeles County.

Principal Responsibilities:

  1. Collaborate effectively with the treatment team to ensure cohesive and comprehensive care.
  2. Conduct intake assessments, establish problem lists, and maintain accurate documentation in coordination with the program coordinator and other team members.
  3. Manage an assigned caseload, providing high-quality treatment services.
  4. Facilitate weekly individual counseling sessions and lead care coordination efforts.
  5. Conduct various counseling groups, including but not limited to: Matrix, Seeking Safety, Parenting, Relapse Prevention, Domestic Violence, Anger Management, Helping Women Recover, Women in Recovery, SUD Education, Life Skills, and Co-Dependency.
  6. Provide detailed progress reports for clients as needed.
  7. Maintain accurate and timely client records using the Electronic Health Record system (PCNX), ensuring all documentation is completed within 24 hours of service delivery.
  8. Collaborate with a multi-disciplinary team that includes the Los Angeles County Probation/Parole Department, Department of Health Services, Department of Public Health, Department of Children and Family Services, and other behavioral and mental health professionals.
  9. Receive and implement feedback to improve treatment approaches and explore innovative strategies.
  10. Ensure client confidentiality in compliance with HIPAA requirements.
  11. Offer immediate support and crisis intervention to clients, ensuring safety and access to appropriate resources.
  12. Engage with clients' families to provide education, support, and foster collaboration in the recovery process.
  13. Assist clients in accessing community resources, including housing, employment, and social services, to support their recovery journey.
  14. Deliver gender-responsive, trauma-informed care coordination in line with SAPC policies, operating within the scope of education, training, and responsibilities.
  15. Identify and address barriers to treatment, providing intensive care coordination as needed.
  16. Provide care coordination services for clients and their children enrolled in the program.
  17. Utilize the Homeless Management Information System for client data input when necessary.
  18. Incorporate mindfulness interventions during care coordination sessions.
  19. Conduct crisis management sessions to address immediate client needs.
  20. Facilitate life skills and other assigned group sessions.
  21. Perform ASAM or Problem List assessments as required.
  22. Administer drug tests in compliance with program protocols.
  23. Prepare detailed reports for DCFS Social Workers and/or Probation Officers.
  24. Monitor client services to ensure adherence to program requirements.
  25. Perform additional duties as assigned.

Requirements:

  1. Certified Substance Use Disorder or Registered (SUD) Counselor: Must be certified or registered by one of the agencies accredited by the NCCA and recognized by DHCS, which include CADTP and CCAPP; and must adhere to all requirements of the California Code of Regulations, Title 9, Chapter 8, and Title 9.
  2. One year of experience as a drug and alcohol counselor.
  3. Knowledge of and training in trauma-informed treatment and evidence-based treatment for substance abuse.
  4. Experience with Electronic Health Record systems.
  5. Experience administering the ASAM, ASI, and other substance abuse assessment tools.
  6. Ability to obtain an NPI number and C number by the time of hire.
  7. Must have a valid California driver’s license, proof of insurance, and an excellent driving record.
  8. Excellent written, oral, organizational, and documentation skills.
  9. Must be a team player, willing to receive feedback and grow professionally.
  10. Ability to perform multiple tasks while demonstrating the capacity to work within a team structure.
  11. Must maintain a current First Aid/CPR certification, physical exam, and tuberculosis test results.

Preferred Qualifications:

  1. Experience in Outpatient treatment working with minority, low income and disenfranchised populations.
  2. Experience and knowledgeable of Drug Medi-Cal (DMC) and Los Angeles County Substance Abuse Prevention Control (SAPC) system of care.
  3. Bilingual capabilities in English and Spanish preferred.
